Special Note To Member of the NYS Empire
Health Insurance Plan:
Your Local Officers Working For
You!!
Good morning.  With Colleen leaving so quickly after the
election, we, as your union officers, have been working
hard to make sure that there is a smooth transition as
Toni takes the office of President.  To that end, we
wanted to give you a little information on Toni’s
background as a union officer and about Diane Billings,
our new 2nd Vice President.

Toni has been a CSEA officer for over 20 years.  She has
held all positions from Shop Steward to President.  Toni
comes from a strong union background.  Born in
Watertown and raised in Winthrop, her father was a
union steward at Reynolds Metals for over 30 years.

Diane comes from an even longer line of unionism in her
family.  Born and raised in Massena, Diane’s dad was a
union employee at Reynolds for over 30 years. Her uncle
was president of the UAW local in Massena for many
years, and when he retired, Diane’s cousin became
president of the UAW.  Diane’s sister is also a Labor
Relations Specialist for CSEA, based out of Watertown.

Union talk around the dinner table was everyday fare for
these girls.

Plan enrollees that all United HealthCare claims for basic major
medical and non-network mental health and substance abuse
services provided in calendar year 2007 must be submitted by

New Participating Provider Web Site:  On January 4, 2008, the
current online Empire Plan participating provider directory
administered by United HealthCare will be replaced with a new site.
Beginning on January 4th, a new and enhanced online provider
directory will be accessible through the URL
www.empireplanproviders.com, and through an updated link on
NYSHIP Online located within the Civil Service website
(
www.cs.state.ny.us/ebd).  Those attempting to log-on to the former
online provider directory, www.provider.uhc.com/empire, on or after
1/4
/2008, will be automatically re-routed to the new site's URL.  This
URL redirection will be available through mid-February.   The new
site offers enhanced capabilities and features over the existing
directory including expanded search options, travel information,
physician specialty definitions, and additional provider background.
The enhanced site also allows users to save, fax, or email provider
search results for viewing in paper or electronic format.  In addition
to locating participating physicians, the new website contains
individual links for locating network Outpatient Surgical Locations
(formerly called Ambulatory Surgery Centers), Laboratories, Provider
Groups, and Infertility Centers.  The site also provides helpful
contact and claim information and links to NYSHIP Online and
MyUHC.com.
